General Information
In order to make sure your visit to the practice goes smoothly, we've put together all the important information about how things work at the practice.

Please fill out the New Patient Intake forms. This will make your visit go a lot faster.

Cancellation Policy

We require 24-hr notice for any appointment cancellation or rescheduling, a $25.00 fee will be assessed if you do not show up or if you make changes within the 24-hr period.

We require 72-hr notice for any procedure cancellation or rescheduling, a $100.00 fee will be assessed if you do not show up or if you make changes within the 72-hr period.

You may be rescheduled if you are 30 minutes late.

For safety reasons, children are not allowed in the office.

Financial Policy

Filing claims to insurance is a courtesy to all patients.

It is the patient’s responsibility to ensure your policy is active on the date of service.

All deductible, co-payments, self-pay payments and balances are due in full at the time of service.

Please bring your medication(s) in the original bottle(s) to every office visit or your appointment will be rescheduled.

Medical records will be released within 3 business days with a completed and signed medical records request. The charge for each copy of records is $20.00 for the first 20 pages and $0.25 for each additional page unless requested by another physician.

Due to high call volume, please do not leave more than one message within a 24 hour period.

Please allow 48 hour prior notice for prescription refills.
